城市健康事工有限公司 (Urban Health Ministry Limited) (下稱 UHM LTD)以服務社
群為本,為大眾的身體和心靈的健康設想,安排不同的非牟利文娛康體健康醫療
活動,促進香港市民健康,並帶領大眾認識基督教,信主耶穌。
【UHM 正形運動】應用平臺 (下稱平臺 )為城市健康事工有限公司 (Urban
Health Ministry Limited) 擁有的資產。

Urban Health Ministry Limited (UHM LTD) serves the community at large with a view
to upholding bodily, mental and spiritual health through non-profit making cultural,
recreational, sports and health activities. UHM LTD also has a mission to spread gospel
in Christianity.
“ UHM Good Posture Exercise ” Application (referred to as “APP”) is the asset owned
by Urban Health Ministry Limited.
1. 請使用者細讀本私隱政策及聲明。本文説明 UHM LTD 如何收集和使用個人
資料。當使用者使用本平臺,即表明該人士已接受本私隱政策及聲明條款。如使
用者不接受本私隱政策及聲明條款,請勿使用本平臺,包括填寫本平臺提供的表
格,填寫任何個人資料,或按下「遞交」按鈕。如已使用,即表示使用者已詳讀,
明瞭和同意接受本私隱政策及聲明條款。UHM LTD 會因應需要而更改本私隱政策
及聲明條款並不會另行通知使用者。使用者如定期使用應留意本私隱政策及聲明
是否有更改。
1. Users are advised to read in details this Privacy Policy Statement. This statement
describes how UHM LTD collects personal information and uses it. When you use this

“APP”, you agree to accept the terms and conditions in this statement. If you do not
accept the terms and conditions of this statement, do not use this “APP”, including
providing personal information on the form provided, or press the “button” for
submission. Once you have provided your personal information and submitted it, you
agree to have read, understand and accepted the terms and conditions in this
statement. UHM LTD will amend, revise, update the terms and conditions regularly
when situations require and will not notify users before or after the revision. Users will
need to observe such amendments and changes on a regular basis when using this
“APP”.
2. 資料收集的聲明和目的
使用者使用本平臺的服務時提交的資料,包括但不限於姓名、性別、聯絡電話、
宗教、電郵、居住地,是自願提供的資料。該資料的收集用途為:
1.1. 身份核証和記錄;
1.2. 溝通和發送有關 UHM LTD 資訊/活動;
1.3. 處理使用者服務要求;
1.4. 收取和管理服務款項;
1.5. 使用者戶口操作和管理。
2. Personal Information Collection Statement and Purpose
Users using this “APP” will need to provide information voluntarily with below
purposes, including but not limited to :
2.1. Validation of identity and record;
2.2. Communication and dissemination relating to UHM LTD information/ activities;
2.3. Handling and processing of Users’ requests;
2.4. Collection and management of service payment;

2.5. Handling and management of Users’ accounts.
3. 香港法例第 486 章 - 個人資料 (私隱)條例
UHM LTD 會遵照香港法例,包括香港法例第 486 章 - 個人資料 (私隱)條例去
收集、使用、保存、披露、移轉、保護、存取和取消個人資料的政策和實施。
使用者同意和授權 UHM LTD 保留和儲存在 UHM LTD 在香港或香港以外的地方的
數據庫内,用作提供、披露、共存取和傳送給 UHM LTD 授權處理該資料的同工、
僱員、技術人員或相關人員。任何披露這些有關資料,必要合乎香港法律、行政
制度或規條、法院命令、已協議的服務條款的要求和必要的情況下才可進行。該
行爲是為保障 UHM LTD,UHM LTD 同工/僱員,使用者相關的權利和利益出發。
UHM LTD 不會銷售使用者資料,亦不會提供、披露、供存取和傳送該資料給未授
權人士。
3. Personal Data (Privacy) Ordinance, Cap. 486, the Laws of Hong Kong
UHM LTD will abide to the Laws of Hong Kong, including Personal Data (Privacy)
Ordinance, Cap. 486 to collect, use, retain, disclose, transfer, maintain, access and
cancel personal information. Users agree and authorize UHM LTD to retain and store
within the data centre in Hong Kong and outside Hong Kong for the purpose of data
facilitating, disclosing, co-assessing, disseminating handled by authorized persons,
employees, technical and related personnel. Any such disclosure will need to follow
the laws, rules and regulations, court orders of Hong Kong and agreed service terms
and conditions. Such provisions are to protect UHM LTD, its workers/employees as well
as the rights and benefits of users during the delivery of services. UHM LTD will not
sell users‘ information and provide, disclose, facilitate access, and disseminate it to
unauthorized personnel.
4. 個人資料安全措施
UHM LTD 會盡力運用各種實際、安全技術和程序來保障使用者的個人資料,避免
其不正當地被使用。但 UHM LTD 不能保證數據傳輸中完全的可靠性。使用者有
權要求更正或刪除其在本平臺的個人資料。如有需要 , 請電郵
infouhm109@gmail.com 或寄通知書到地址:香港北角渣華道 128 號渣華商業中
心 1305 室謝小姐收。
本平臺不會收集 18 嵗以下的未成年人士的個人資料。如未成年人士使用本平臺
時應由父母陪同和監督。

4. Personal Information Security Measures
UHM LTD will try its best to apply practical, safety technology and procedures to
protect and safeguard personal information, preventing the illegal use of it. But UHM
LTD will not guarantee the reliability of data transmission. Users have the right to
request for revising or deleting their personal information in this “APP”. If this is
required, please email to infouhm109@gmail.com or mail notification to below
address:
1305 , Java Commercial Centre, 128, Java Road, Hong Kong (Attention to Miss Tse).
This “APP” will not collect personal information for users under 18 years of age. If these
underaged users want to use this “APP”, they need to be escorted and guided by their
parents.
5. 資訊包(cookies)
UHM LTD 有權使用資訊包(cookies)。資訊包是網絡服務器儲存在用戶電腦中的
簡短文字文件, 作爲用戶瀏覽器軟體所設置的許可。資訊包本身不會用於讀取
用戶的電腦上任何的資訊, 只用作網站使用的識別功用。使用者可以對網絡瀏
覽器進行調整,向其發出警告或是拒絕接受資訊包。但這做法會令至本平臺無法
提供服務給使用者,或不允許使用者登錄。
5. Cookies
UHM LTD has the right to use cookies in this “APP.” Cookies are small text files stored
on the users’ computer by a web server based on the permission set by the users’
browser software. Cookies themselves are not designed for the purpose of accessing
any information in the users’ computer. They are to identify browsed websites placed
on users’ hard disk. Users can adjust the browser in their computer, to send to the
browser alerts or declines to accept cookies. But doing so may render this “APP”
unable to provide services to you or register you as user.
6. 第三方網站或應用程式的鏈結
UHM LTD 不對第三方網站應用程式負責。UHM LTD 提議使用者先向任何第三方
網站或其應用程式詳細瞭解其處理私隱的做法。
6. Links to Third Party’s Websites or Applications

UHM LTD shall not be responsible to third party’ websites or applications. UHM LTD
recommends users to read and understand the privacy practices of any such third
parties’ websites or applications before any information is provided to them.

本文為【UHM 正形運動】應用平臺 (下稱平臺 )的使用條款及細則。
請使用者使用本平臺前詳細閱讀本文的條款。使用者使用本平臺,包括下載、存
取平臺的内容時即表示使用者接受本文的使用條款。如使用者不接受使用條款的
約束,請勿使用本平臺。

Terms and Conditions

This article specifies the terms and conditions for the use of “UHM Good Posture
Exercise ” Application (referred to as “APP”).
Users are advised to read and understand in details the terms and conditions of this
“APP”. When users are using this “APP“ , including downloading, accessing
contents of this “APP” , agree to accept its terms and conditions. If you do not want
to be bound by the terms and conditions, please do not use this “APP”.
1. 使用條款
【UHM 正形運動】應用平臺乃是城市健康事工有限公司(Urban Health Ministry
Limited)(下稱 UHM LTD)擁有的資產。本條款適用於任何使用本平臺的使用者。
UHM LTD 保留修改本使用條款的權利。使用者使用本平臺是為同意接受使用本平
臺的條款或其已更新版本的約束。UHM LTD 不容許任何人使用本平臺作任何非法
目的或用途上, 包括但不限於是威脅、中傷、誹謗、淫穢、色情、褻瀆、鼓吹
不當、刑事、違反法規等行爲。在未經 UHM LTD 事先書面同意下,任何人不得張
貼或傳送任何有關銷售或商業活動資訊到本平臺上。
1. Use of Terms and Conditions
“UHM Good Posture Exercise” Application (“APP) is the asset owned by Urban Health
Ministry Limited (UHM LTD). These terms and conditions are applicable to users to this
“APP”.

UHM LTD reserves the right to revise theses terms and conditions. On accessing to this
“APP”, users agree to accept the terms and conditions including its revised version.
UHM LTD do not allow users of this “APP” to conduct any acts with illegal means and
purposes, including but not limited to threatening, libel, defamation, obscenity,
pornography, blasphemy, improper advocacy, criminality, illegality etc. Without the
prior permission of UHM LTD, nobody is allowed to post or disseminate any
information for acts of sale or commercial activities on this “APP“.
2. 免責聲明和責任限度
2.1 UHM LTD 隨時更新本平臺的資訊,並不會另行通知使用者。資訊有效期
是以 UHM LTD 的伺服器的時間爲準。在任何情況下,UHM LTD 不保證資訊的準
確性,完整性,可靠性或及時性。UHM LTD 不會負責使用者使用本平臺需要的相
關設備、流動數據服務、wifi 無綫網絡的費用等;或能否順利或順暢在使用者的
手機、硬體或設備上運作。
2.2. UHM LTD、其負責人、董事、僱員、第三方服務提供者不會保證使用者使
用本平臺時發生的以下情況。使用者應注意自行承擔責任和相關風險, 提供適
當和足夠保護、備份、適時的病毒掃描、偵察破壞性或損害性因素的檢測。情況
如下:
2.2.1. 平臺能否時常開啓;
2.2.2. 符合使用者要求;
2.2.3. 使用平臺時不被中斷,不會有任何延誤、故障、錯誤、遺漏或失去任何
傳送中的訊息;
2.2.4. 平臺有病毒入侵、產生病毒或其引致破壞的情況;
2.2.5. 不負責連於 UHM LTD 第三方軟件的效能和安全。
2.3. 在任何情況下,UHM LTD、其負責人、董事、僱員、第三方服務提供者不
會承擔使用本平臺時引起的任何直接,間接的損失、傷害、賠償或索償。
2. Disclaimer and Limitation of Liability
2.1. UHM LTD will revise the information of this “APP” regularly without notifying

users before or after the revision. The valid period of information is subjected to the
time set in the server. In any circumstances, UHM LTD will not assure the accuracy,
completeness, reliability and timeliness of the information. UHM LTD is not
responsible for user’s need for related facilities, mobile data services, wifi service fees
etc; or smooth operation of users’ electronic devices, hard disks or equipment.
2.2. UHM LTD, its responsible persons, directors, employees, third party service
providers will not guarantee below occurrences. Users are responsible for their own
liabilities and risks and to provide appropriate and adequate protection, back up,
timely virus scanning, detection tests for destructive or damaging factors that are risky
to their own equipment. These occurrences include:
2.2.1. Can the “APP” be opened regularly;
2.2.2. Compatibility to users’ needs;
2.2.3. Suspension, delay, interruption, error, omission during usage and
transmission;
2.2.4. Virus invasion, detection or other destructive situations.
2.2.5. UHM LTD is not responsible for the links to third parties software
applications, whether they are effective, safe or not.
2.3. In any circumstances, UHM LTD, its responsible persons, directors, employees,
third party service providers are not responsible for any direct or indirect losses,
damages, compensations or claims arising from the use of this “APP”.
3. 知識產權
UHM LTD 擁有或已獲授權使用本平臺所載的有關資料的知識產權(包括但不限於
文字、圖像、照片、聲帶、音樂、視頻、軟件、商標、原始代碼等)。如經 UHM
LTD 同意,UHM LTD 會容許使用者根據 UHM LTD 條款,在不損害所有知識產權及
其他所有權的情況下下載本平臺的資料,作爲個人或非商業用途。如未經 UHM
LTD 事先以書面同意的情況下,嚴禁任何人作任何複製、翻版、再造、上傳、下
載、發佈、發放、傳輸、廣播、串流、存儲本平臺有關資料或作任何形式傳閲等
的用途上。

3. Intellectual Property
UHM LTD owns or is authorized to use this “APP” ‘s contents with intellectual property
(including but not limited to texts, images, pictures, sound tracks, music, videos,
software, patents, source codes etc.). Users require UHM LTD’s permission to use this
information in accordance with the terms and conditions for the purpose of personal
or non-commercial purposes. Without the written permission of UHM LTD, it is
prohibited for anybody to copy, pirate, re-produce, upload, download, publish,
disseminate, transmit, broadcast, do streaming, access for aforesaid purposes the
contents of this “APP”.

5. 連接和保安
【UHM 正形運動】應用平臺會載有其他第三方營運的應用程式、網址、網頁的
連結。對於這些第三方的連結的内容、如何使用、管理使用者的個人資料,
UHM LTD 不能管控。本平臺使用者應先瞭解第三方的連結,明白第三方如何使
用, 管理使用者的個人資料。UHM LTD、其負責人、董事、僱員、第三方服務
提供者不會承擔任何因使用該些第三方連結所引致的損失。
5. Links and Security
“UHM Good Posture Exercise” Application contains links to third party software
applications, websites and website pages. UHM LTD has no control of how to manage
the contents and use of these third party links. UHM LTD recommends users to
understand these links in full details relating to the use of personal information
before connecting to these links. UHM LTD, its responsible persons, directors,
employees, third party service providers are not responsible for any damages or
losses arising from usage of these third party links.
6. 賠償和豁免

使用者是為同意在不抵觸法律的情況下使用本平臺。使用者因違反本使用條款
所引致對 UHM LTD、其負責人、董事、僱員或第三方服務者賠償或使其受任何損
害, UHM LTD 會保留權利刪除或拒絕提供使用者資訊或内容,或對使用者發出警
告,暫停或終止其使用本平臺。UHM LTD 也會保留申索因使用者涉嫌違反本使
用條款的調查而引致的損失。使用者同意不能因其違反本使用條款引致 UHM LTD
的上述反應行動而對 UHM LTD、其負責人、董事、僱員、及第三方服務提供者提
出索償。
6. Compensation and Exemption
Users agree to use this “APP” in accordance with the Laws of Hong Kong. If users have
violated the terms and conditions of this “APP” and caused UHM LTD, its responsible
persons, directors, employees, third party service providers any claims or damages ,
UHM LTD reserves the right to delete, decline provision of information provided to the
“APP“,issues warnings, suspends or terminates users’ usage of the “APP”. UHM LTD
reserves the right to claim users for any costs incurred by investigating into users’
violations of the terms and conditions. Users agree not to claim UHM LTD, its
responsible persons, directors, employees, third party service providers for any
compensation as a result of their breach of “APP”s terms and conditions.
7. 適用法律
本使用條款受制於中華人民共和國香港特別行政區的法律,並須按照香港特別行
政區的法律解釋。
7. Governing Law and Jurisdiction
These terms and conditions are governed and interpreted by the Laws of the Hong
Kong Special Administrative Region of the People’s Republic of China.
8. 條款及細則差異
中英文版本的條款及細則如有任何差異,概以中文版本為準。
8. Discrepancy in Interpretation
If there is any discrepancy between the Chinese and English versions of these terms

and conditions, the Chinese version shall prevail.

款及細則

使用者登記成為本平臺使用者,是爲同意已經閲讀並理解本條款約束。

“UHM Good Posture” Application (referred to as “APP”) –
Users’ Terms and Conditions

Users of this “APP” agree to abide by the terms and conditions of this “APP”.

1. 本條款之適用性
本條款是為使用者與 UHM LTD 之間的協議。如有需要,UHM LTD 可在未經事先
通知使用者的情況下修改本條款。
1. Applicability of These Terms
These terms are an agreement between UHM LTD and the user to this “APP”. If
situation arises, UHM LTD will revise these terms without prior notification to the
users.
2. 【UHM 正形運動】應用平臺使用者登記
2.1. 登記成爲本平臺使用者, 申請人必須:
2.1.1. 安裝【UHM 正形運動】應用平臺於設備内;

2.1.2. 填寫表格内列明必須的項目;
2.1.3. 在其提供的電郵收到 UHM LTD 的信息後確認;
2.1.4. 保證及聲明在登記時所提供的資料均屬真實無誤和完整。
2.2. 如登記後要更改資料,使用者要在本平臺的【我的賬戶】内進行更新。
2.3. UHM LTD 保留權利可以拒絕,暫時終止,撤銷任何登記申請。
2. “UHM Good Posture Exercise” Application – Users’ Registration
2.1. A user who wants to register this “APP” is required to:
2.1.2. Complete the form, fill out the required items;
2.1.3. Acknowledge registration in user’s provided email message sent from UHM
LTD;
2.1.4. Assure and claim accuracy and validity for information completed.
2.2. If revision is needed after registration, update in the “My Account” in the
“APP”.
2.3. UHM LTD reserves the right to decline, suspend, terminate the registration.
3. 本平臺帳戶及密碼的安全與保安
3.1. 已登記的使用者有責任安全地保管其登記的賬戶和密碼。如有以下情況,
應聯絡 UHM LTD 負責人謝小姐(電郵:infouhm109@gmail.com) 要求
立即停止已登記的賬戶:
3.1.1. 遺失安裝本平臺賬戶的設備;
3.1.2. 上述設備遭盜竊;
3.1.3. 已登記的賬戶被他人使用或賬戶密碼被披露予非授權人士。

3.2. UHM LTD 不會負責因上述情況(3.1.1.、3.1.2.、3.1.3.)直接或間接引致有關
的任何損失或損害。
3. User ‘s Account and Password – Safety and Security
3.1. After registration, users are responsible to securely keep the account and
password. If below situations occur, please notify Miss Tse (Email :
infouhm109@gmail.com) to terminate the account:
3.1.1. The device with the “APP” is lost;
3.1.2. The device with the “APP” has been stolen;
3.1.3. Unauthorized use of the account and password;
3.2. UHM LTD is not responsible for above situations (3.1.1., 3.1.2., 3.1.3.), whether
they incur direct or indirect losses or damages to users.
4. 個人資料
使用者使用本平臺,必須提供登記表格要求的「必須填寫」的個人資料。沒
有注明「必須填寫」的部分,使用者可以自由選擇提供。UHM LTD 不會承擔
資料更新的準確性和完整性的任何責任。
4. Personal Information
Users using this “APP” need to complete “Required” information. Users are free to
provide information for the “optional” items. UHM LTD is not responsible for the
accuracy and completion of the updated information.
5. 一般條款
5.1. 使用者同意和確認對已登記的賬戶負責。并不會出讓或授權第三方使用
該賬戶。使用者同意使用本平臺時遵守所有適用的法律和條款。如要取消已登
記的賬戶,請聯絡謝小姐(電郵:infouhm109@gmail.com)。在以下情況之下,
UHM LTD 無需提前通知,可以暫停、取消或終止使用者已登記的賬戶:

5.1.1. 使用者違反本條款;
5.1.2. UHM LTD 有合理理由相信使用者違反本條款。
5.2. UHM LTD 保留追討使用者違反本條款所引致任何對 UHM LTD, 其負責
人、董事、僱員或第三方服務提供者造成的傷害和損失的權利。
5. General Terms
5.1 Users agree and acknowledge to safekeep their accounts. Users are not to sell
or authorize third parties to use the accounts. Users agree to abide by the terms and
conditions of this “APP”. For any cancellation of accounts, please notify Miss Tse
(Email : infouhm109@gmail.com). UHM LTD will not notify in advance any
suspension, cancellation or termination of users’ accounts under following
circumstances:
5.1.1. Users contravening these terms and conditions;
5.1.2. UHM LTD reasonably suspects users contravening these terms and
conditions;
5.2. UHM LTD reserves the rights to claim users contravening these terms and
conditions, resulting in the losses and damages incurred to UHM LTD, its responsible
persons, directors, employees, third party service providers.
6. 適用法律
本使用條款受制於中華人民共和國香港特別行政區的法律,並須按照香港特別行
政區的法律解釋。
6. Governing Law and Jurisdiction
These terms and conditions are governed and interpreted by the Laws of the Hong
Kong Special Administrative Region of the People’s Republic of China.
